- New clients seeking a consult should download the Client Consent Form
- We encourage you to make time read thru and understand the form before signing
- The consent form should be filled completely and emailed back to the email mentioned in the form
- Once we have received the signed consent form and payment, within 24 hrs. we will work with you to setup consult time
- The fee includes the suggested doses of single remedies that you may need, brief check-in calls/emails/Text to clarify the remedy plan
- Mailing/shipping cost of anything that is sent from my office is paid on actuals
- Links and suggestions will be provided for remedies I don’t have, gemmos, cell salts, vitamins and supplements, if part of the plan are paid seperately
- If you are experiencing financial difficulty, please reach out to discuss and arrange a mutually workable fee.
- We provide “Good Faith Estimates” upon request
Interim Support
- For scheduling, brief check-ins, and clarifications of the current plan, please tex/call or email. There is no charge for such communication and are intended to be brief and able to be taken care of in 5 minutes or less.
- If an acute appointment (usually 15 minutes) is required, it can often be scheduled later that day or conducted via email, and are chargeable.
Payments
- The client agrees they are responsible for payment of all fees once the consult time is scheduled
- Please note that calls/emails/text exchanges asking for a remedy counts as a consultation and will incur an aproportioned charge
- Payment may be made via Stripe or Bank Transfers (Payment Links are in the Consent Form)
- A $40 fee will be charged for any check returned to this office unpaid or in case there is a delay in payment
Cancellations: Life happens
- If it is necessary to cancel/reschedule an appointment, an email notification of schedule change must be made at least 48 hours in advance
- The Client is responsible for payment of consult fees for no-shows, and late cancellations with less than 24 hours notice.
